Joining Our service & How to access it!

BECOME A MEMEBER/ SERVICE USER!

We are always looking for new Members (service users), If you are looking for something to do and meet people like yourself. We have service Users with Autism & Asperger's.

We run multi-activity groups run by volunteers, We meet Ones a month on on 1st Tuesday from 2pm - 4:30pm At Community Hub by Compassion Acts, 17A Cambridge Walks, Southport PR8 1EN. Any problems or concerns or just an enquire please don't hessite to contact one of our team members. 


No referral Needed to attend our hubs, Just turn up, fill out an application form via our website or a paper version on your first visit! We have a joining fee/ subs as we are We are independently run and we need the money to help us. we are not for profit group. The money we get goes back into the hub to buy equipment, supplies for the hub. We not about making money! Its about supporting our service users and doing things for them to get them independent!

 

We do offer a referral system, If have referred by one of Our Partners please let us know. If you are referring a New service user to our service please complete the referral application form below. Please state in the application what service you are referring from. If your service keeps referring new service uses to us on regulate basis we could look at setting up a partnership with your self/ Organisation.    We Currently don't have a waiting list!

Action Plan/ Care Plan

New Service users that come to our service that have additional needs we will do an Action Plan/ Care Plan with them. This ensures our service users get the right level of carer and support when they are under our care and support. It also shows from an outsider looking in we are doing our jobs correctly and right. We will sit down with the service user and maybe their support worker if they have one and do the plan together as the Action Plan/ Care Plan is all about them. We will visit the Action Plan/ Care Plan every 3 to 4 months to see if there is any changes or updates to their health or conditions. This Action Plan/ Care Plan gets signed of by the coordinator who does the Action Plan/ Care Plan and the service user and if they have a support worker too! The Action Plan/ Care Plan will very for each service user as they are difference and not all our service users will have an Action Plan/ Care Plan in place! If you think you need might need an Action Plan/ Care Plan putting in place, please speak to one of the coordinators or volunteers on duty!

Joining fee

From January 2024 we are introducing subs. (£1 a month or £12 Year, comes with Membership Card)

This will help us pay towards to the room hire & Refreshments due to the cost of the cost of living crisis. We are a small community organisation and rely on donations and funding to keep us going. 

 

If you pay £12 a year you will get a receipt for your payment.You can pay cash or bank Transfer as we have a bank account for the hub!
