New Service users that come to our service that have additional needs we will do an Action Plan/ Care Plan with them. This ensures our service users get the right level of carer and support when they are under our care and support. It also shows from an outsider looking in we are doing our jobs correctly and right. We will sit down with the service user and maybe their support worker if they have one and do the plan together as the Action Plan/ Care Plan is all about them. We will visit the Action Plan/ Care Plan every 3 to 4 months to see if there is any changes or updates to their health or conditions. This Action Plan/ Care Plan gets signed of by the coordinator who does the Action Plan/ Care Plan and the service user and if they have a support worker too! The Action Plan/ Care Plan will very for each service user as they are difference and not all our service users will have an Action Plan/ Care Plan in place! If you think you need might need an Action Plan/ Care Plan putting in place, please speak to one of the coordinators or volunteers on duty and we can have a 1 to 1 section / chat. If we don't have the time we can book you in for a 1 to 1 with a male or female volunteer or coordinators.
